Radiation-induced pathological fractures of the proximal femur: a case series considering an endoprosthetic solution

چکیده
ABSTRACT BACKGROUND: Radiation-induced pathological fractures of the proximal femur are difficult to treat due frequent non-union and hardware failure using standard fracture fixation techniques. This case series investigates endoprosthetic replacement as a treatment option METHODS: A retrospective folder review from private hospital in Cape Town, patients who had sustained radiation-induced fracture, was reviewed descriptive statistics. RESULTS: Six met inclusion criteria. One patient excluded minimum follow-up time six months not met. Of Ave that were analysed, all transverse, subtrochanteric fractures. Prior definitive with femoral replacement, three treated trauma instrumentation prior referral unit, one vascularised fibular graft their initial while at unit. an first procedure Among graft, there 100% rate. ablation free musculocutaneous flap periprosthetic infection after replacement. only complication At median 15 (min 7, max 55) Musculoskeletal Tumour Society score 74% 63%, 93%. CONCLUSION: seeks highlight high rates seen when treating this condition or biological methods. Further research is needed, but may be viable alternative solution Level evidence: 4. Keywords:

Bilateral Endoprosthetic Replacements of the Proximal Femur
Patient. We report the case of a 20-year-old caucasian male with an Ewing's sarcoma in the left upper femur. After induction chemotherapy, he underwent resection of the left upper femur and insertion of a cemented proximal femoral replacement. Four years later, he presented with a solitary bone metastasis in the right upper femur. He underwent further chemotherapy followed by resection and endo...
متن کاملSurgical Treatment of Pathological Fractures Occurring at the Proximal Femur
PURPOSE To analyze the results of surgical treatment for pathological fractures at the proximal femur. MATERIALS AND METHODS Nineteen patients with a pathological fracture were included. The mean age was 65.7 years old. The patients comprised 8 males and 11 females. Primary tumors, types of pathological fractures, surgical procedures, and postoperative complications were recorded. Musculoskel...
متن کاملSafety aspects in surgical treatment of pathological fractures of the proximal femur – modular endoprosthetic replacement vs. intramedullary nailing
BACKGROUND Pathologic fractures of the femoral intertrochanteric and subtrochanteric region require special consideration in terms of biomechanically stable fixation and durability of the implant. In addition, the type of surgery might also influence patient survival. We conducted this retrospective study to evaluate the safety of modular proximal femur replacement compared to intramedullary na...
متن کاملEndoprosthetic replacement for giant cell tumour of the proximal femur.
PURPOSE To evaluate the functional and oncological outcomes of 12 patients with giant cell tumour (GCT) of the proximal femur treated with customized endoprosthesis. METHODS Nine men and 3 women aged 26 to 52 (mean, 36) years with Campanacci stage-III GCTs of the proximal femur were included. All underwent a wide excision of the tumour with clear margins and replacement using a customized, bi...
متن کاملModular endoprosthetic replacement for metastatic tumours of the proximal femur
BACKGROUND AND AIMS Endoprosthetic replacements of the proximal femur are commonly required to treat destructive metastases with either impending or actual pathological fractures at this site. Modular prostheses provide an off the shelf availability and can be adapted to most reconstructive situations for proximal femoral replacements. The aim of this study was to assess the clinical and functi...
